You are more then welcome to complete the forms in the office; however, please arrive 15 minutes prior to your scheduled appointment so you have time to complete them.
If you are on a complex medication and/or supplement regimen, please be sure to include everything you are taking.
The first session lasts an hour so we can review your health history, where you currently are, and develop a treatment strategy designed specifically for you. Follow-up visits generally last about forty-five minutes.

Please bring any information about your case that you may have received from your other doctors such as X-ray/MRI statements, lab test, blood work, etc. As with any medical visit, it's a good idea to write down questions you may have ahead of time and keep a running list so that you don't forget anything you'd like to address or ask. I'm an advocate of patient education, therefore no question is too inconsequential or off the table.

- Eating a large meal within one hour or your appointment
- Fasting for more than six hours before your appointment
- Alcohol, tobacco, food or drinks that color your tongue (such as coffee) immediately prior to treatment
- Vigorous exercise or sexual activity within the hour before treatment
CANCELLATION POLICY
Appointments are time slots set aside specifically for you. In fairness to both new and returning patients, the clinic maintains a 24-hour cancellation policy. You may be asked to provide a credit card number (including 3 digit CVV code) when scheduling; this will ONLY be used if canceling within 24-hours, in which case you will be charged for a full treatment.
Insurance Coverage
We accept all insurance that covers Out-of-Network acupuncture treatments. If you are unsure of your policy, please complete the Insurance Verification form below below prior to your first visit so that I may confirm the extent of your coverage with your carrier.
Do you offer any discounts?
We offer reduced rates for students, military and public service personnel, and senior citizens. Please bring in a current/active identification to your first visit to receive the discount.
For those who do not have insurance coverage for acupuncture, we also offer a prompt-pay treatment package that reduces the overall cost of treatments. Please inquire for more information.
